Photography & Video Release

By signing this release, I permit Carlsbad Educational Foundation to use and publish photographs and/or video of my child(ren) for the purpose of promoting the work of these entities. This includes, but is not limited to materials used for educational, promotional, commercial, or fundraising purposes. I specifically decline payment or other consideration for granting these rights. I expressly waive any claims I may have against Carlsbad Educational Foundation, or its affiliates, officers, directors, employees, or agents, except as outlined in writing herein. By signing this release, I allow Carlsbad Educational Foundation to update my permissions in ProCare, if applicable, to match the permissions granted below. By signing the line below, I represent that I am at least 18 years of age and that I have read and understand this Release.
